Description

The Basketball Disciples Academy (BDA) 12-Week Player Development Program is designed for boys and girls who are committed to improving their game through consistent, high-quality training.

Each week follows a progressive curriculum that builds upon the previous week’s instruction, allowing athletes to continually develop new skills while reinforcing proper fundamentals. Our experienced coaches focus on developing complete basketball players by combining skill development, athletic performance, basketball IQ, and live game application.

Throughout the program, athletes will receive instruction in:

  • Ball Handling & Dribbling
  • Passing & Court Vision
  • Shooting Mechanics & Shot Development
  • Finishing at the Rim
  • Footwork & Offensive Moves
  • Defensive Fundamentals
  • Basketball IQ & Decision Making
  • Strength, Conditioning & Agility
  • Team Concepts & Communication
  • Competitive Game Play

Every training session includes:

  • Dynamic warm-up and athletic development
  • Fundamental skill instruction
  • Position-specific development
  • Basketball IQ and situational learning
  • Competitive drills and live play
  • Character development emphasizing discipline, respect, effort, leadership, and teamwork

At the conclusion of the 12-week program, each athlete will participate in a comprehensive skills evaluation to measure progress and identify areas for continued growth.

Whether your athlete is just beginning their basketball journey or preparing for competitive school or AAU basketball, this program provides the foundation necessary for long-term success on and off the court.

Recommended Ages: 8–17 (Athletes are grouped by age and skill level.)

Program Length: 12 Weeks

Training Schedule:

  • 2 sessions each week
  • Saturday and Sunday
  • 1 week day lab
  • 3 hours per session

Mission: Develop fundamentally sound basketball players while building confidence, character, discipline, and a lifelong love for the game.

MEDICAL AUTHORIZATION

I certify that my child is physically capable of participating.

I understand Basketball Disciples Academy does not provide medical insurance.

In the event of an emergency where I cannot be reached, I authorize Basketball Disciples Academy staff to obtain emergency medical treatment deemed necessary.

I understand that I am solely responsible for all medical expenses incurred.

I authorize emergency medical treatment for my child if I cannot be reached and accept responsibility for all medical expenses.

WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK

NOTICE TO THE MINOR CHILD’S NATURAL GUARDIAN

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF JC ELITE PRO LLC, DOING BUSINESS AS BASKETBALL DISCIPLES ACADEMY, AND THE OTHER RELEASED PARTIES IDENTIFIED BELOW USE REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED.

BY AGREEING TO THIS FORM, YOU ARE GIVING UP YOUR CHILD’S RIGHT AND YOUR RIGHT TO RECOVER FROM JC ELITE PRO LLC, DOING BUSINESS AS BASKETBALL DISCIPLES ACADEMY, AND THE OTHER RELEASED PARTIES IDENTIFIED BELOW IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY.

YOU HAVE THE RIGHT TO REFUSE TO AGREE TO THIS FORM, AND JC ELITE PRO LLC, DOING BUSINESS AS BASKETBALL DISCIPLES ACADEMY, HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT AGREE TO THIS FORM.

1. Program Activities

I am the parent or legal guardian of the athlete being registered. I give permission for my child to participate in the Basketball Disciples Academy 12-Week Player Development Program and related activities, including basketball instruction, conditioning, stretching, running, jumping, agility work, strength and athletic-performance exercises, drills, practices, scrimmages, games, evaluations, clinics, camps, tournaments, and related activities.

2. Assumption of Risk

I understand that participation in basketball and athletic training involves inherent risks and dangers that cannot be completely eliminated, even when reasonable safety precautions are used.

These risks may include, but are not limited to:

  • Contact or collisions with other athletes, coaches, equipment, walls, floors, basketball goals, or other objects;
  • Falls, slips, trips, overexertion, dehydration, heat-related illness, or fatigue;
  • Sprains, strains, fractures, cuts, bruises, dental injuries, concussions, head or spinal injuries, illness, permanent disability, or death;
  • Injuries caused by the actions, inaction, negligence, or intentional conduct of another participant; and
  • Other risks that are inherent in basketball, conditioning, competition, and athletic training.

I knowingly and voluntarily assume these risks on behalf of myself and my child.

3. Release and Waiver

To the fullest extent permitted by Florida law, I release and waive any claim or cause of action that I or my child may have for personal injury, death, or property damage resulting from the inherent risks of participation against:

  • JC Elite Pro LLC;
  • Basketball Disciples Academy;
  • Their owners, members, officers, directors, employees, coaches, trainers, volunteers, contractors, agents, affiliates, and sponsors;
  • YMCA of Southwest Florida and the applicable YMCA facility;
  • Facility owners, operators, landlords, tournament organizers, and event hosts; and
  • Each of their respective employees, representatives, agents, and affiliates.

Collectively, these persons and entities are referred to as the “Released Parties.”

This release applies to claims arising from the inherent risks of the activities described above, including risks that remain even when the Released Parties use reasonable care. Nothing in this waiver is intended to release liability that cannot legally be waived under Florida law.

4. Medical Fitness and Emergency Care

I represent that my child is physically able to participate or that I have disclosed any medical condition, limitation, allergy, or medication that may affect participation.

If an illness or injury occurs and I cannot be reached promptly, I authorize Basketball Disciples Academy personnel to contact emergency services and obtain reasonably necessary emergency medical care for my child.

I understand that:

  • Basketball Disciples Academy does not provide health or accident insurance for participants;
  • I am responsible for medical, ambulance, hospital, and related expenses incurred for my child; and
  • Academy personnel may remove my child from an activity when they reasonably believe continued participation may be unsafe.

5. Acknowledgment of Electronic Agreement

By selecting the checkbox below, I confirm that:

  • I am the athlete’s parent or legal guardian and have authority to agree on the athlete’s behalf;
  • I have read and understand this entire Waiver, Release of Liability, and Assumption of Risk;
  • I understand that this agreement affects important legal rights;
  • I voluntarily accept its terms on behalf of myself and my child;
  • I consent to completing this agreement electronically; and
  • My electronic acceptance is intended to serve as my signature
