Rookie Basic is our year-round basketball development membership created specifically for young athletes ages 5–7 who are beginning their basketball journey.
Athletes train one day per week in a positive, energetic, and age-appropriate environment designed to introduce the fundamentals of basketball while developing coordination, confidence, listening skills, teamwork, and a love for the game.
Training may include:
• Basic ball handling and dribbling• Passing and catching• Shooting fundamentals• Footwork and body control• Balance, coordination, and agility• Introduction to defense• Basketball rules and court awareness• Teamwork and sportsmanship• Fun competitive drills and age-appropriate games
Our goal at this level is not simply to teach basketball skills. We want young athletes to become comfortable on the court, learn how to follow coaching instruction, develop good movement habits, and build a strong foundation for future basketball development.
Basketball Disciples Academy operates year-round with rolling enrollment, so athletes may join at any time.

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF JC ELITE PRO LLC, DOING BUSINESS AS BASKETBALL DISCIPLES ACADEMY, AND THE OTHER RELEASED PARTIES IDENTIFIED BELOW USE REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED.
BY AGREEING TO THIS FORM, YOU ARE GIVING UP YOUR CHILD’S RIGHT AND YOUR RIGHT TO RECOVER FROM JC ELITE PRO LLC, DOING BUSINESS AS BASKETBALL DISCIPLES ACADEMY, AND THE OTHER RELEASED PARTIES IDENTIFIED BELOW IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY.
YOU HAVE THE RIGHT TO REFUSE TO AGREE TO THIS FORM, AND JC ELITE PRO LLC, DOING BUSINESS AS BASKETBALL DISCIPLES ACADEMY, HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT AGREE TO THIS FORM.
1. Program Activities
I am the parent or legal guardian of the athlete being registered. I give permission for my child to participate in the Basketball Disciples Academy 12-Week Player Development Program and related activities, including basketball instruction, conditioning, stretching, running, jumping, agility work, strength and athletic-performance exercises, drills, practices, scrimmages, games, evaluations, clinics, camps, tournaments, and related activities.
2. Assumption of Risk
I understand that participation in basketball and athletic training involves inherent risks and dangers that cannot be completely eliminated, even when reasonable safety precautions are used.
These risks may include, but are not limited to:
I knowingly and voluntarily assume these risks on behalf of myself and my child.
3. Release and Waiver
To the fullest extent permitted by Florida law, I release and waive any claim or cause of action that I or my child may have for personal injury, death, or property damage resulting from the inherent risks of participation against:
Collectively, these persons and entities are referred to as the “Released Parties.”
This release applies to claims arising from the inherent risks of the activities described above, including risks that remain even when the Released Parties use reasonable care. Nothing in this waiver is intended to release liability that cannot legally be waived under Florida law.
4. Medical Fitness and Emergency Care
I represent that my child is physically able to participate or that I have disclosed any medical condition, limitation, allergy, or medication that may affect participation.
If an illness or injury occurs and I cannot be reached promptly, I authorize Basketball Disciples Academy personnel to contact emergency services and obtain reasonably necessary emergency medical care for my child.

Parent/Legal Guardian Authorization
I am the parent or legal guardian of the minor athlete being registered with Basketball Disciples Academy. I understand that photographs, video recordings, audio recordings, or other digital media may be captured during Basketball Disciples Academy practices, training sessions, evaluations, games, scrimmages, camps, clinics, tournaments, special events, and other academy-related activities.
I authorize JC Elite Pro LLC, doing business as Basketball Disciples Academy (“BDA”), and its authorized representatives to photograph, record, and/or capture the image, likeness, voice, and athletic participation of my child in connection with Basketball Disciples Academy activities.
1. AUTHORIZED USE
I authorize Basketball Disciples Academy to use, reproduce, edit, publish, display, distribute, and share approved photographs, videos, recordings, and other media featuring my child for legitimate academy-related purposes, including:
• Basketball Disciples Academy websites and online platforms• Official social media accounts• Promotional and marketing materials• Flyers, brochures, advertisements, and digital graphics• Email and electronic communications• Player-development and training content• Highlight videos and promotional reels• Team and academy announcements• Community outreach and sponsorship materials• News, media, and public-relations purposes• Livestreams or recordings of games, practices, tournaments, or academy events• Other lawful promotional or educational purposes related to Basketball Disciples Academy
2. PRIVACY AND IDENTIFICATION
Basketball Disciples Academy will make reasonable efforts to protect the privacy of minor athletes.
Unless separately authorized or reasonably necessary for team, competition, recognition, or player-profile purposes, Basketball Disciples Academy will not intentionally publish a minor athlete’s sensitive personal information, including home address, personal telephone number, personal email address, school schedule, or other private identifying information.
This authorization permits the use of my child’s image or likeness individually or as part of a group.
3. OWNERSHIP AND COMPENSATION
I understand that photographs, videos, recordings, graphics, and other media created or commissioned by Basketball Disciples Academy may become the property of JC Elite Pro LLC or the applicable content creator, subject to applicable law and contractual rights.
I understand that neither my child nor I will receive compensation, royalties, or other payment for the authorized use of such media.
4. EDITING AND PUBLICATION
I understand that photographs and recordings may be cropped, edited, formatted, combined with graphics, music, text, logos, or other content, and adapted for different media platforms, provided the use is lawful and consistent with the purposes described in this authorization.
Basketball Disciples Academy will not knowingly use an athlete’s image or likeness in a defamatory, unlawful, or intentionally misleading manner.
5. RELEASE
To the extent permitted by law, I release and hold harmless JC Elite Pro LLC d/b/a Basketball Disciples Academy and its owners, members, officers, employees, coaches, trainers, volunteers, contractors, agents, and authorized representatives from claims arising solely from the authorized capture, publication, reproduction, or use of my child’s image, likeness, voice, or appearance as permitted by this Media Release.
Nothing in this authorization is intended to waive rights or claims that cannot legally be waived under applicable law.
6. DURATION AND WITHDRAWAL OF AUTHORIZATION
This authorization will remain in effect while my child participates in Basketball Disciples Academy programs and activities unless I revoke my authorization in writing.
I understand that revocation will apply to future uses after Basketball Disciples Academy has had a reasonable opportunity to process my request. Revocation may not require Basketball Disciples Academy to retrieve, remove, destroy, or discontinue materials that were lawfully created, published, distributed, printed, posted, or provided to third parties before the revocation was received and processed.
7. ELECTRONIC CONSENT
By selecting the applicable consent option and electronically submitting this registration, I confirm that:
• I am the athlete’s parent or legal guardian and have authority to provide this authorization;• I have read and understand this Photo, Video & Media Release Authorization;• I understand how my child’s image, likeness, voice, and recordings may be used;• I voluntarily provide the authorization indicated through the registration system; and• My electronic acceptance is intended to serve as my electronic signature.
