The IRON SHARPENS IRON 1V1 TOURNAMENT is a fast-paced, competitive basketball event designed to challenge players, build confidence, and highlight individual skill. Open to both boys and girls, this one-day tournament gives athletes the chance to compete head-to-head and see how they measure up against top talent in the area.

Who Can Participate

The tournament is open to players in grades 3 through 12. Athletes will be grouped into skill-appropriate divisions to ensure fair and competitive matchups.

Divisions

Division 1: 3rd & 4th Graders
Division 2: 5th & 6th Graders
Division 3: 7th & 8th Graders
Division 4: 9th – 12th Graders

All divisions are co-ed, and players will compete against others in the same grade bracket.

d/b/a BASKETBALL DISCIPLES ACADEMY

TOURNAMENT PARTICIPATION AGREEMENT, MEDICAL AUTHORIZATION, CONCUSSION ACKNOWLEDGMENT, ASSUMPTION OF RISK, LIABILITY WAIVER & MEDIA RELEASE

This Agreement applies to participation in the JC Elite Pro LLC d/b/a Basketball Disciples Academy (“JC Elite Pro”) Iron Sharpens Iron 1v1 Basketball Tournament and all related tournament activities, warm-ups, competitions, awards, and activities conducted in connection with the event.

By registering a minor participant and accepting this Agreement, I represent that I am the participant’s parent or legal guardian and have authority to provide the permissions and agreements below.

1. MEDICAL CONDITION AND FITNESS TO PARTICIPATE

I represent that, to the best of my knowledge, the participant is physically capable of participating in basketball activities.

I agree to disclose to JC Elite Pro any known medical condition, allergy, medication, physical limitation, previous injury, or other condition that could reasonably affect the participant’s safe participation.

I understand that JC Elite Pro, Basketball Disciples Academy, its coaches, staff, and representatives do not provide medical diagnoses or guarantee that a participant is medically fit to participate.

JC Elite Pro may restrict or discontinue a participant’s participation when reasonably necessary for the participant’s safety or the safety of others and may require appropriate medical clearance before permitting the participant to return.

2. EMERGENCY MEDICAL AUTHORIZATION

If I cannot be reached in the event of an injury or medical emergency, I authorize JC Elite Pro, Basketball Disciples Academy, its coaches, staff, representatives, and event personnel to contact emergency medical services and obtain or arrange reasonably necessary emergency medical care for the participant.

I authorize emergency transportation by ambulance when reasonably necessary.

I understand that JC Elite Pro cannot guarantee the availability or outcome of medical treatment.

I accept financial responsibility for medical treatment, emergency transportation, hospital services, and other medical expenses incurred on behalf of the participant to the extent not otherwise covered by insurance.

3. CONCUSSION AND HEAD INJURY ACKNOWLEDGMENT

I understand that participation in basketball involves a risk of concussion and other head injuries.

A concussion is a brain injury that may result from a blow to the head or body and may occur even without loss of consciousness.

Possible signs or symptoms may include headache, dizziness, nausea, balance problems, confusion, memory difficulty, vision problems, sensitivity to light or noise, unusual fatigue, behavioral changes, or other neurological symptoms.

I understand the potential danger of continuing athletic participation following a suspected concussion or head injury.

I agree that the participant must immediately report concussion symptoms or a suspected head injury to a coach, official, staff member, parent, or other responsible adult.

I understand and agree that a participant suspected of sustaining a concussion or head injury will be immediately removed from participation and will not be permitted to return to athletic activity until the requirements of applicable Florida law have been satisfied, including appropriate written medical clearance when required.

4. ASSUMPTION OF RISK

I understand that basketball and competitive athletic activities involve inherent risks that cannot be completely eliminated regardless of the care taken to avoid injuries.

These risks include, but are not limited to:

• Running, jumping, stopping, cutting, pivoting, and physical exertion;

• Falls and loss of balance;

• Contact or collision with other participants;

• Contact with basketballs, basketball goals, backboards, walls, floors, bleachers, equipment, officials, coaches, spectators, or other objects;

• Sprains, strains, bruises, cuts, fractures, dislocations, dental injuries, and eye injuries;

• Dehydration, heat-related illness, fatigue, and overexertion;

• Concussions and other head injuries;

• Serious bodily injury, permanent disability, and, in rare circumstances, death.

I understand these risks and voluntarily permit the participant to participate in the tournament.

5. FLORIDA MINOR PARTICIPANT NOTICE AND LIABILITY RELEASE

NOTICE TO THE MINOR CHILD'S NATURAL GUARDIAN

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF JC ELITE PRO LLC d/b/a BASKETBALL DISCIPLES ACADEMY, ITS OWNERS, AFFILIATES, EMPLOYEES, OR AGENTS USE REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED.
